Vitality Precision Health (“VPH”) is a wellness coaching and concierge lab-analysis service that is not a HIPAA-covered entity. However, we voluntarily comply with the standards of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) to protect your health data. This Notice explains how we collect, use, share, and protect your protected health information (PHI).

What Information We Collect

We may collect and store the following information:

Your name, date of birth, and contact details

Symptom intake responses

Laboratory results retrieved from Evexia Diagnostics (with your consent)

Genetic results retrieved from SelfDecode or Sequencing.com (with your consent)

Payment information via Stripe or PayPal

How We Use and Share Your Health Information

Use of Your Information:

We use your information solely to create your personalized Vitality Report™ and provide optional coaching support.

To analyze and interpret your lab and genetic data

To generate your report using secure, HIPAA-enabled Azure OpenAI (with Business Associate Agreement in place)

To process payments and communicate with youFor customer service and administrative purposes

Disclosure of Your Information:

We only share your PHI with:

Evexia Diagnostics (for lab orders and results)

SelfDecode (for DNA interpretation) or Sequencing.com

HIPAA-enabled Azure OpenAI environment for report generation (raw data may be uploaded; not de-identified)

Vendors under signed Business Associate Agreements (e.g., encrypted cloud storage)

Legal or regulatory authorities if required by law

Account Creation & Data Custody

If you opt in to the WGS upgrade, your Sequencing.com account will be created and maintained under our master account for the purpose of ordering, processing, and delivering your genome data.

This setup means Vitality Precision Health will have full account-level access to your genomic data, including the ability to download, store, and process your raw data files.

Data Use

Your genomic data will be used solely for the purpose of providing the agreed-upon health coaching and analysis services.

We will not share your raw data with any third party without your explicit written consent, except as required by law.

Data Storage & Security

Your data will be stored securely on Sequencing.com’s servers and within our private encrypted storage systems.

Sequencing.com’s privacy and security policies also apply to your data; you may request a copy of these policies at any time.

Data Access, Transfer, and Deletion

You may request a copy of your raw genomic data at any time.

You may request that we delete your genomic data from our systems and/or Sequencing.com, or transfer your data to a separate account in your name.

Upon such a request, we will act promptly, subject to any legal or regulatory requirements for retention.

Acknowledgment of Different Processing Method

You understand that the Sequencing.com account setup differs from our standard DNA processing service, and that Vitality Precision Health will serve as the account holder with Sequencing.com on your behalf.

You consent to this arrangement for the purposes of your WGS Upgrade.

Your Privacy Rights

You have the right to:

Request a copy of the information we hold about you

Request corrections to your data

Withdraw your consent for data use at any time (applies to future processing only)

Request deletion of your stored information (except where we’re legally required to retain it)

File a complaint if you believe your privacy rights have been violated
