Welcome to our Precision Health Coaching practice. These Terms and Conditions (“Terms”) explain the scope of our services, outline each party’s responsibilities, and include important legal and ethical disclaimers. By purchasing The Vitality Report or engaging in our coaching packages, you acknowledge that you have read, understood, and agree to these Terms.

Scope of Services

The Vitality Report provides personalized wellness insights based on your DNA, lab results, health history, symptoms, and goals. It offers lifestyle recommendations, supplement ideas, and wellness strategies to support your vitality.

We also offer optional 3- or 6-month coaching packages to help you implement the report’s recommendations. Coaching is designed to support behavior change, accountability, and goal setting. We provide information, tools, and motivation, but you remain responsible for your health decisions. Coaching is not clinical treatment or therapy.

Scope of Practice & Limitations

We are not licensed healthcare providers.

We do not diagnose, treat, or cure diseases, prescribe medications, or provide medical advice. Our services are educational and wellness-focused, intended to complement—not replace—licensed medical care.

Always consult your physician or qualified healthcare provider before making major changes to your diet, exercise, supplements, medications, or treatment plans. Our coaching is designed to empower you with information, but implementation is voluntary and at your discretion.

If any clinical concerns arise during coaching, we will refer you to your appropriate healthcare providers.

Not Medical Advice – Important Disclaimers

Nothing in our services, reports, website, emails, or communications should be considered medical advice, a substitute for medical advice, or a guarantee of specific results.

We do not claim to prevent, diagnose, treat, or cure any disease or medical condition. We strongly recommend you seek proper medical evaluation for any concerns. Do not change medications or healthcare routines without your doctor’s supervision.

Your participation is voluntary, and you assume full responsibility for any outcomes resulting from implementing suggestions we discuss.

Informed Consent & Client Responsibilities

By engaging our services, you affirm that:

You understand the nature and limitations of health coaching.

You voluntarily consent to participate.

You are solely responsible for your health decisions and outcomes.

You will communicate openly and honestly to facilitate the coaching process.

You will seek licensed healthcare services as needed.

You agree that implementation of any ideas discussed is at your own risk and based on your informed judgment.

Use of Lab Results and Genetic Data

We may incorporate your lab results or genetic data into The Vitality Report. These results are used for informational purposes only. We do not independently verify lab data and do not interpret results in a diagnostic or clinical capacity.

We treat all your data with confidentiality and use it solely to personalize your wellness recommendations. You agree to share data voluntarily and acknowledge that we are not liable for the accuracy or security of third-party lab results.

Privacy, Confidentiality, and Data Protection

Your privacy is extremely important to us.

Confidentiality: We do not share your personal information without your consent, except as required by law.

Data Storage: Client data may be stored securely on FGFunnels and similar HIPAA-compliant or GDPR-compliant platforms.

Limitations: While we use strong safeguards, no technology platform is 100% secure. By using electronic communications, you accept minor inherent risks.

HIPAA: We are not a HIPAA-covered entity but voluntarily uphold high privacy standards.

Client Expectations and No Guarantee of Results

Health outcomes depend on many factors, including individual biology, lifestyle, commitment, and circumstances. We do not guarantee specific results.

We offer guidance, structure, and support, but you are ultimately responsible for your own progress.

Past client success stories are illustrative only and are not a promise or prediction of your experience.

Fees, Payment, and Refund Policy

Full payment is required at the time of purchase unless otherwise agreed.

Refund Policy:

24-hour full refund window (minus 5% processing fee).

No refunds after Vitality Report work has begun or after 24 hours have passed.

Coaching packages are nonrefundable once sessions have begun, though unused sessions may be refunded pro-rata at our discretion for major life events.

Rescheduling Policy:

24-hour notice required for session changes.

Missed sessions without notice are forfeited.

Liability Release and Limitation of Liability

By using our services, you agree to release, indemnify, and hold harmless our company, coaches, and representatives from any liability related to your participation, except in cases of gross negligence.

You accept full responsibility for your actions, choices, and outcomes resulting from coaching and the Vitality Report.

Any disputes will be governed by Florida law.

Governing Law and Dispute Resolution

These Terms are governed by the laws of the State of Florida. Any disputes will be handled through good-faith negotiation, and if necessary, through legal action in Florida courts.
